The learning outcomes for the conference are as follows;
  • Recognise changes in cognition in older people (both reversible and irreversible) and be able to assess cognition in an older person.
  • Identify the modifiable risk factors that increase the risk of cognitive dysfunction.
  • Understand how commonly prescribed medications affect the older person.
  • Outline the key role of the nurse in supporting and educating patients and their families about their medication.
  • Describe the assessment, treatment and management of atrial fibrillation in older people.
  • Undertake a stroke risk assessment and bleeding risk assessment in a person prescribed blood thinning medication for atrial fibrillation.
  • Describe the key components of integrated care.
  • Recognise the role of each multi-disciplinary professional in integrated care and how an integrated care model could be applied in your workplace.
